Record Activity Attendance

Whether electronically or handwritten it is important to record the activity attendance of the individuals that you serve.  Recording activity attendance provides documentation to support efforts to keep individuals active and engaged in their preferred activities.

There are multiple formats to records activity attendance.  The following information fields should be included in your activity record so that you can examine it for quality improvement or care planning processes.

Name, Date, Activity, Preference Category, Engagement Score, Facilitator, Location
